Liability-Release

CAMP: In order for your child to attend Surf Camp or Ocean Adventure Camp, this form must be filled out in it's entirety and returned to our office no later than two weeks before the start of camp. If we do not receive this form, your child will not be able to participate! For legal reasons, we cannot accept these forms at the beach - please return it to our office in one of three ways:

PRIVATE LESSONS/GROUP EVENTS: You do not need to fill this form out in advance; you will have the chance to do so at check-in.
